What are weekends?

Peds has been a challenge for me but I’m enjoying it. I see about 15-18 pts on my own & then discuss my assessment/plan with my preceptor. There’s often little time to discuss pts so I’ve gotten better at presenting relevant findings. I’ve also been getting my practice in reading EKGs & X-rays. Monkey bars are surprisingly the cause of a lot of injuries (haha who knew 🤸🏽‍♀️)

The kids seem to love my DIY pumpkin badge..yay for sparkly distractions! 🎃Anyway, that’s all for now. Happy Saturday ❤️

Hello everybody! As our discussion on dermatological complaints continues, here is a glimpse of what bullous pemphigoid is.
Age of onset: 60s and over
Presentation: Itchy, urticated, erythematous and tense bullae.
Pathology: Autoantibodies develop against the hemidesmosomal antigens. Antigen-antibody binding initiates complement activation and inflammation, hemidesmosomal damage and consequent blistering.
The bullae are tense as the other strata above are intact. These bullae are referred to being in a subepidermal location, aptly.
Diagnosis: History, clinical examination followed by immunofluorescence on salt-split skin. IF displays a subepidermal and LINEAR IgG and C3 deposition at the basement membrane zone.
One more thing we should remember is because these are subepidermal, they DO NOT involve MUCOUS MEMBRANES.
Treatment:
Since it affects the elderly, topical steroids may be sufficient, considering co-morbidities.
Systemic steroids such as prednisolone not greater than 0.75mg/kg/day.
Oral Doxycycline helps limiting usage of steroid due to its anti-inflammatory properties, often combined with Azathioprine.
Evidence to support the use of IV Immunoglobulin, again as a steroid saving therapy.

Now todayy was a cool sessionn! As you can see all of us vividly listening to Rob (practicing PA) explain chest leads for a 12-Lead ECG (Electrocardiogram- measures electrical activity of your heart❤).
What's the first thing you notice? (Besides that ✌🏻from our "patient") - It's Rob's sleeves- bare below the elbows! This is clinically important to have nothing, including jewellry below the elbows before conducting a examination. Wash your hands, introduce yourself and explain and gain consent about the procedure. OFFER A CHAPERONE! Very important to do this especially in a intimate physical examination like this.
Getting to the cool bits... the sticky pads do not just go anywhere on the chest; you have to know your anatomy for it. Saying "5th intercostal space, midclavicular line" may sound alien to some but that is the exact anatomical position for one of the leads and also the apex beat of your heart (the 'pointy' bit of the heart that hits your chest when it beats). There are leads that go on both your arms and legs that are not clear in the picture but they are there. The leads(wires) are attached to the sticky pads on the patient. In total, there are 6 chest leads (V1-V6) and 3 limb leads of which the other 3 are "virtual leads" so not physically attached. (12 Leads in total)
Fun fact: the limb lead on the right foot/ankle actually is a neutral lead and doesn't measure any activity of your heart! 😱

Because the sticky pads were not as efficient, we're going to have to try again next week to do it properly. And hopefully obtain a ECG trace.
Also thanks to our volunteer patient. After which, he had erythema (redness- me getting all clinical) on his chest from taking the sticky pads off. We told him it was a free waxing session for him - a thank you. Poor him but he was a sport!

One of the hardest things to do is to tell your loved ones you have cancer. Not only do you have the shock of receiving the news yourself and processing it. But then you start worrying about how it will affect your family and friends. A lot of people have told me they worry for others than themselves and feel guilty for burdening them with it. Here is how my friend found that first 24 hours of being a ‘cancer patient’. "The biopsy results are back … and it has shown … it is cancer. What?!! I actually have cancer! Me?!! Why? What have I ever done wrong in life to deserve cancer. I’m not a bad person, why is this happening to me? I couldn’t help those feelings, I took a moment to put my head down and cried. I have cancer! I looked up but all I heard was mastectomy and chemotherapy. The moment you are told it feels like a bomb has gone off and you’re in the middle of a massive explosion, people are talking but you can’t hear them, everything is in slow motion. Happy Fri-Yay and end of PA week everyone! I wanted to thank everyone for following me on my ig and those of y'all who read my blog ❤❤ Much love. I'm always so surprised by the number of people who think my life is interesting because at the end of the day, I'm just another girl trying to make it in this world. In honor of The end of PA week, I'm going to address some points that get brought up a lot when I'm asked why I chose the PA profession. 1️⃣ I CHOSE to go to PA school! Not because I couldn't get into med school, and no I don't think PA school is "settling" at all. I respect med students so much and their path! But for me becoming a PA allowed me to reach my personal and career goals in life in a way that fit me best. 2️⃣ Someone once told me that becoming a PA was like becoming a "glorified nurse" and "under appreciated doctor." UHHH 🙄🙄 Okay, I think nurses are awesome and incredibly patient humans and do so much and doctors are so dedicated! I've personally never met a PA who didn't LOVE their job! And now being on rotations, it's only proven that I am so so so happy just being there with patients and helping them with their problems! There's definitely high job satisfaction in this profession. 👩🏻‍⚕️So yeah, just two points I'd like to make. But, I'm working on a little project right now that'll give you more insight on health professions and why we chose what we chose!
