Your intervertebral discs are NOT like jelly donuts. Have you ever seen a jelly donut squat 1000lbs, dunk a basketball, or run 25 miles per hour? Me neither.

Did you know that up to 66% of lumbar disc herniations, of any variety, can actually demonstrate healing with conservative treatment? In fact, the worse the herniation, the better the spontaneous regression might be!
.
However, imaging does not necessarily need to show resorption of the disc for symptoms to improve. Research also suggests that specific loading through exercise may have the potential to impart healing or regeneration of the intervertebral disc.

Surgery has its place, but understand that your body is resilient, you often CAN get better with time, and you can never go wrong getting strong through exercise.

💥Tight Quads!?💥

Here is a typical progression on how you can attack tight quads as a warm-up prior to working out.
_
The exact exercises don’t matter here. It’s the thought process and order of events that matters.
_
1️⃣Soft tissue mobilization of the “tight” structures.
_
Foam roll the quad for 30-90 seconds.
_
It isn’t enough to make sustainable change to the tissues, but does excite the nervous system and give you a temporary improvement in mobility.
_
2️⃣Mobilize the area.
_
I am a fan of the couch stretch to further improve your mobility.
_
When stretching, we like to add gentle PAILs/RAILs.
_
3️⃣Actively strengthen the new range of motion.
_
Foam rolling and stretching may temporarily give you more mobility, but adding strength and actively controll to your end range is where it’s at!
_
Skipping this step will ensure you don’t improve your active mobility, leaving yourself at greater risk for injury.
_
In the final video, we are using End Range Lift Offs for hip extension, utilizing knee flexion to lengthen the quad.
_
The goal is to lift your knee as high as possible without dropping the lacrosse ball, or letting the lower back arch.
_
Give these a try, and remember this order can be utilized for any area of the body.

DISC HERNIATIONS CAN HEAL

One of the issues I get the most questions on is low back pain, especially disc herniations. While there is a lot that goes into this question, two things I usually mention are that disc herniations are common in much of the asymptomatic population and that they can heal with time and conservative treatment.
.
The following is a case report from the New England Journal of Medicine that documents the resolution of a lumbar herniation without surgery.
.
📕A 29-year-old woman presented to the spine clinic with new-onset pain in her right leg, accompanied by paresthesia. MRI of the lumbar spine revealed a lumbar disc herniation resulting in substantial spinal stenosis and nerve-root compression (left image).
.
📕She elected conservative treatment with physical therapy and an epidural injection of glucocorticoids. A second MRI obtained at follow-up 5 months after presentation showed resolution of the herniation (right image). Her clinical symptoms resolved, and she was discharged from the clinic, with follow-up recommended as needed.
.
📕Data from clinical trials suggest that patients who have herniated lumbar discs have similar long-term outcomes whether they undergo surgery or elect conservative management. In addition, the risk of subsequent catastrophic worsening without surgery is minimal.

Operation Day - 11/01/2018

Today is the day. When I did my injury on 23rd August in a light training session I was devestated. I was playing on a 3G surface in firm ground blade football boots when I crumbled to the floor. The 5 months that followed meant light rehab work and psychological preparation for my operation day. I had a private MRI scan which revealed a torn ACL, a meniscus tear, LCL tear, a hairline fracture to my femur and severe bone bruising to my tibia. Not good.
Operation day rolled away fast and I was told that the operation should take 2 hours. It took 3. Shit. Prompt anxiety increase. I was first in theatre. I was told my surgery would be a double bundle style reconstruction allograft, who h having looked at the NICE guidance, is a very strong and up to date graft.
My surgeon was fantastic. As a nurse I was told excellent things from staff who worked alongside him. Regaining consciousness I remember seeing him in recovery and his words were "I am fairly happy with how the surgery went Chris". I had a nerve block, which didn't work I didn't think. The next few hours involved me drifting in and out of sleep albeit in pain. Which was the order of the day with an elevated leg. I would say the pain is a 9/10 at this point with the knee very achey, although I was surprised that it wasn't necessarily the knee drill sites that hurt and more the hamstring tendon site.
